Band 8 on the NHS Agenda for Change pay scale is split into four sub-bands -- 8a, 8b, 8c and 8d -- covering senior managers, advanced clinical practitioners, senior specialist leads and heads of department, with combined pay spanning roughly £53,000 at the bottom of 8a to around £99,000 at the top of 8d, before reaching the single Band 9 executive tier above. Taking a representative Band 8a salary of £56,000 for 2026/27: taxable income after the £12,570 Personal Allowance is £43,430, with £37,700 taxed at 20% (£7,540) and the remaining £5,730 taxed at 40% higher rate (£2,292), giving total Income Tax of £9,832. National Insurance is 8% up to the £50,270 Upper Earnings Limit (£3,016) plus 2% on the £5,730 above it (£114.60), totalling £3,130.60. Combined deductions of £12,962.60 leave £43,037.40 take-home pay a year, around £3,586.45 a month. NHS Pension Scheme contributions at Band 8 are tiered by actual pensionable pay and typically sit around 10.7% to 12.5%, deducted before Income Tax, so the true reduction in take-home pay is partly offset by the tax relief this generates.
