Clinical coders translate patient records into standardised ICD-10 and OPCS-4 codes that determine NHS hospital funding and support national health data. It is a detail-oriented, office-based (often hybrid or remote) NHS role sitting on Agenda for Change Bands 3 through 7. Pay ranges from around GBP 24,625 for a Band 3 trainee to GBP 52,809 at the top of Band 7 for a clinical coding manager. This guide sets out realistic UK pay by NHS band, shows estimated take-home after Income Tax, National Insurance and NHS Pension contributions for 2026/27, and explains the qualification route into the role. Most clinical coders join the NHS as Band 3 trainees and complete the national Clinical Coding Qualification -- covering ICD-10 diagnosis coding and OPCS-4 procedure coding standards -- typically over 12--18 months of structured training and assessment. Passing the qualification and the annual national coding standards refresher is usually required to progress to Band 4 and beyond, and further experience and specialist coding audit skills support progression to Band 6 senior/approved coder and Band 7 clinical coding manager roles.
Clinical coding is also one of the more remote-friendly NHS roles, with many trusts offering hybrid or fully remote working once a coder is qualified and experienced -- a factor that some coders value as highly as salary progression.
Clinical coders are eligible for the NHS Pension Scheme (2015 CARE scheme), a defined benefit arrangement accruing 1/54th of pensionable pay each year, revalued by CPI+1.5%. Employee contributions are tiered: 5.2% up to GBP 13,259, 6.5% up to GBP 26,831, 8.3% up to GBP 32,691, 9.8% up to GBP 49,078, 10.7% up to GBP 62,924, and 12.5% up to GBP 111,377. Most clinical coder salaries fall in the 6.5%--9.8% tiers. The employer contributes approximately 23.7% of pensionable pay on top -- a substantial benefit not reflected in take-home pay figures.
