NHS physician associates (PAs) are employed under the Agenda for Change (AfC) pay framework, entering at Band 7 in recognition of their postgraduate qualification, with progression to Band 8a for senior and lead roles. Since December 2024, PAs are regulated by the General Medical Council. Salaries range from GBP 46,124 for a newly qualified PA to GBP 60,504 at the top of Band 8a. Physician associates became subject to statutory regulation by the General Medical Council (GMC) from December 2024, under the Anaesthesia Associates and Physician Associates Order 2024. This introduced a formal register, defined fitness-to-practise processes, and a clearer scope of practice, following a period of significant scrutiny of PA supervision arrangements, role boundaries and patient safety in NHS settings.
PAs work under the supervision of a named senior doctor and support diagnosis, management and continuity of care across a wide range of specialties including general practice, emergency medicine, surgery, psychiatry and internal medicine. They are a distinct regulated profession from doctors, with a different training pathway (typically a postgraduate PA Master's following an undergraduate degree) and a different pay structure under AfC rather than the medical and dental pay scales.
PAs in the NHS Pension Scheme (2015 CARE scheme) pay tiered employee contributions based on pensionable pay -- broadly 9.8% to 10.7% for most Band 7 PAs, rising toward 12.5% at Band 8a. Contributions are usually collected via salary sacrifice, reducing taxable pay, so the after-tax cost is lower than the headline percentage suggests.
As a defined-benefit scheme, the NHS Pension provides retirement income linked to career average earnings rather than investment performance, and its actuarial value is often estimated at an additional 20--25% of salary over a full NHS career -- making it one of the most valuable elements of total compensation for PAs, alongside the base salary shown in the tables above.
